Sciatica Case Study

A 53 year old male truck driver presented to clinic suffering from a two month history of persistent nerve pain down the back of the left leg without any lower back pain.

A 53 year old male truck driver presented to clinic suffering from a two month history of persistent nerve pain down the back of the left leg without any lower back pain. Upon questioning the pain began after waking one morning and cannot recall any particular incident that may have caused it. He mentions having a history of similar episodes in the past that resolved on its own with time, however this episode appears to remain persistent. The patient found it difficult to bear weight on his leg while walking and was unable to sit for long periods of time. Stretching, pain killers and rest were unable to resolve his pain. No pins and needles, numbness or weakness were mentioned.

Examination findings:

    • Reproduction of leg pain with neural stress tests
    • Neurological examination – normal – no changes to sensation, strength or deep tendon reflexes
    • Tender to touch along the gluteal, hamstring and calf muscles
    • Pain with walking and sitting
    • Range of motion – pain reproduction with forward bending

Diagnosis:

    • Left sided S1 radiculopathy (Sciatica) due to underlying L5/S1 disc bulge

Treatment Plan:

    • Reduce pain along the leg via decompression therapies for the lower back and gluteal muscle
    • If no further improvements made, refer for MRI/CT and potential referral for cortisone injection

Treatment:

    • 2x/week over 4 weeks
    • Massage, dry needling, low level laser therapy to affected areas
    • Spinal manipulation to thoracic spine
    • Home stretching exercise/stretching routine
    • Anti-inflammatory medication

Outcome:

    • 70% reduction in Sciatic pain along the leg
